Mit dem Traum-Lineup mit Chris Reifert, Eric Cutler, Danny Coralles und Joe Trevisano liefern Autopsy eine wahnsinnige Mischung aus Mid-Tempo Death Metal, gepaart mit Doom-Elementen, schneidenden Twin-Guitars und absolut kranken Vocals ab.
Als Besinnung auf ihre Ursprünge gibt es eine Coverversion von »Fuck You« (Bloodbath).
»Puncturing The Grotesque« wurde in den Earhammer Studios in Oakland aufgenommen unter der Aufsicht von Greg Wilkinson und das Artwork stammt von Dennis Dread (Darkthrone, Abscess).
